FEEDBACK FORM

This form is a space for you to give us any feedback, thoughts or suggestions on how the board and committees/teams are doing!

 

We are here to listen to you so don't be scared to write any comment you have : )

It can be what we did good, what we did wrong, what we should do etc. There is nothing useless for us!!!

 

None of the questions are mandatory, they are there as guidance but if you have something specific you would like to add then just write it in the open question at the end. 

 

The form is completely anonymous, however, if you would like to leave your name or contact details please free to do so :)

 

Also if you would like to discuss anything with us in person, come chat during the board's walk-in hours or send us an email/text/insta pm!

Reimbursement FORM

This form allows  for committees to request funds for expenses incurred during their activities.

The form should include fields for the committee name, date of the event, description of the expense, and the amount requested. It's important to ensure that all receipts and supporting documentation are attached to the form to avoid misunderstandings and fraud
